Facial plastic and aesthetic surgery

The outer appearance should harmonise with our experience and our consciousness, i.e. our “inner life”, so that we also feel comfortable “in our skin”.

We want an outward appearance that is at least largely acceptable to us. Or we simply want to look as young as we feel.

If we are not satisfied, we consider changing something, e.g. through a healthy lifestyle, sport, sufficient sleep, a healthy diet – and also surgical measures that improve our outer appearance. This is because these can also contribute to a higher quality of life.

Ästhetische Chirurgie und reconstructive surgery are basically non-identical twin sisters because they use similar techniques. Oral and maxillofacial surgeons who have great expertise in reconstruction are therefore also particularly gifted aesthetic surgeons. This is because, in addition to correcting minor whims and imperfections of nature, it is always important to “restore” the youthful, fresh appearance.

Patient videos – plastic and aesthetic surgery

An oral and maxillofacial surgeon has particularly detailed anatomical knowledge of the head, neck and face. He knows the lines of the bone structure, the muscles and the covering soft tissues particularly well. He is not only an expert in all aspects of the corresponding skin and soft tissue conditions, but also a specialist in reconstructive surgery (e.g. after tumour removal or after ageing processes such as skin sagging). We will be happy to advise you on which operations can be covered by your health insurance and which operations you (at least as a holder of statutory insurance) will be liable to pay for yourself. For all measures subject to co-payment, you will first receive a detailed cost estimate. Skin tightening procedures can be performed under sedation or local anaesthesia on an outpatient basis. Inpatient monitoring is only advisable for certain conditions. A general anaesthetic can also be given, especially if you wish.

Ear correction / otoplasty

If there is a desire to reduce the size of the ears or to flatten protruding ears (“bat ears”), this is done in children and adults by means of an ear correction (medical term: otoplasty). This can be done from around the age of five, when the outer ear has finished growing. Today’s surgical procedures are very gentle with very good results, and the procedure is usually completed in one day. Usually both ears are corrected, even if only one sticks out or is too large, to ensure good symmetry.
Depending on the age and condition of the patient, the procedure is performed under local or general anaesthetic.
The operation takes about 45 to 60 minutes per ear. First, the surgeon makes an incision behind the ear. This gives him access to the ear cartilage, which is then corrected as desired by means of cutting, grinding or suturing techniques, depending on the type of malposition. In the case of protruding ears, the longitudinal bulge in the middle ear cartilage is usually missing, so this is then surgically reshaped. What remains is a small scar behind the ear, which fades after six months at the latest and is thus inconspicuous. After the bandage is removed, the patient is given a headband to stabilise the position of the ear.

Blepharoplasty

Upper eyelid blepharoplasty corrects upper eyelids that look like drooping eyebrows due to light exposure and age-related skin sagging. Drooping upper eyelids also change facial expressions, making the face look tired, unfriendly and absent-minded. A correction thus not only ensures a rejuvenated appearance, but also makes the facial expression look more present, alert and friendly. During the planning of an eyelid plastic surgery, it should be differentiated whether the eyelid skin is too flabby, the entire eyelid is too bulky or whether disturbing humps (often on the inner side) are causing the otherwise slender upper eyelid to bulge out. The same surgical access route is chosen for all these irregularities. But skin, muscle tissue and fat are reduced in combination to varying degrees.

If the lower eyelid looks too wrinkled and forms “bags”, only a little skin and muscle tissue needs to be reduced. Here, the incision is chosen directly below the eyelashes and is almost invisible after healing. If only one “lacrimal sac” is bulging, incisions are made from the inside of the eyelid and the fatty tissue that has prolapsed in these areas (the term “lacrimal sac” has nothing to do with the tear glands) is removed.

Profile corrections of the nose

Profile enhancements on the face are usually carried out in combination with a nose and chin or lower jaw line correction, sometimes also with a cheekbone correction if no jaw or teeth position correction is to be carried out at the same time (see also nose and profile corrections). These procedures must be performed as in-patient or day-case procedures and under general anaesthesia.

Further aesthetic corrections

The brow lift raises the eyebrows and smooths the skin of the upper third of the face. We use dissolving hooks that make the skin tension finely adjustable. The skin incision has regained its former firmness four weeks postoperatively, and the body has completely broken down the hooks after about two years without losing the firming effect. The usual incision along the hairline, even if offset into the hairline, for a forehead lift or complete (round-face) facelift, should not be used for men, as they tend to show more and more scalp as they age due to hormonal factors such as typical male hair loss. Here we usually reduce the forehead skin directly above the eyebrows, along the course of a small skin fold.

Here, the entire face is rejuvenated and tightened via hidden incisions according to your needs and wishes. The facelift is almost always preceded by an eyelid lift, after which the lower face and mid-face are also individually rejuvenated and tightened either as a “mini-lift”, “mid-facelift” or classic complete facelift.

This is done under sedation accompanied by an anaesthetist. Usually you can go home afterwards after a recovery phase in our relaxation room, as long as a reliable person looks after you (spouse/life partner, family member, good friends). Of course, you will be given an emergency telephone number for any questions and problems and you can also visit us again at any time at the emergency outpatient clinic of Sana Klinikum Offenbach.

Medicinal facial skin rejuvenations with so-called “fillers” or in the form of wrinkle treatment with botulinum toxin can be performed on an outpatient basis. While fillers correct skin wrinkles by, as the name suggests, “filling in” and padding under wrinkles, botulinum toxin weakens the muscles, which then prevent the skin from wrinkling by permanently relaxing them. Like all aesthetic surgery procedures, these treatments are excluded from the catalogue of health insurance benefits and must be paid for by the patient. In your own interest, please clarify whether the costs will be covered by your (statutory or private) health insurance. Unfortunately, we are unable to provide extensive correspondence with your health insurance company.

For all aesthetic and other procedures at your expense, we handle payment via the billing company DZR, which offers you the option of convenient payment in instalments.

For patients with statutory health insurance, the billing of self-pay services via their health insurance card is generally excluded. Should your health insurance company be willing to cover the costs in a special case, this is tied to the reimbursement regulations of your health insurance company.
